John Peterson Special Needs Scholarship

Cleveland Scholarship

Dear Parent(s),

It’s now time to apply and/ or renew for your child(ren's) state scholarship. Student's may only receive one state scholarship during any given school year.

St. Mary offers the following state scholarships:

• EdChoice/ EdChoice Expansion Scholarship (All families can apply!) (Every family that applies for EdChoice/ EdChoice Expansion will receive funds to support private school tuition in Ohio)

• Cleveland Scholarship (Families who reside in Cleveland can apply)! (Funds received will be directly applied to private school tuition if the school is a provider of the scholarship)

• Jon Peterson Special Needs Scholarship (Students who have an IEP/ Individualized Education Plan, can apply to the Jon Peterson scholarship. Funds received will pay for the private school tuition and/ or services rendered to support the student's need based on the IEP)

NEW to EDCHOICE or the EDCHOICE EXPANSION SCHOLARSHIP? Follow these steps: (links are found below)

1. Download and complete your 2024/25EdChoice Expansion Scholarship Request formper each child.
2. Complete the EdChoice Income Verification Process. This form and supporting documents need to be emailed to the ODE. (Drop box is provided by the ODE on their website. Only one (1) Income Verification is needed from families for the first time you apply).
3. Families who have multiple children enrolled at St. Mary school, please complete a separate EdChoice/ EdChoice Expansion Request form for each child.
4. Provide Proof of Residency dated within 90 days from the submission date for the scholarship. (See the link below for more information regarding Proof of Residency).
5. Provide a copy of your child(ren's) birth certificate.
6. All documents, EXCEPT for the Income Verification form, are to be sent to Mr. Crawford, the SMS Scholarship Coordinator, via email ([email protected]) or in person to the school office.
7. The ODE will review you EdChoice/ EdChoice Expansion Scholarship Request form once you have completed it. The ODE will contact you should any additional documents or information needs to be corrected.

RENEWING the EDCHOICE / EDCHOICE EXPANSION SCHOLARSHIP (links are found below)

1. Download and complete the EdChoice Renewal form
2. Provide Proof of Residency, dated within 90 days of submission date. (For more information regarding Proof of Residency, please refer to the link below).
3. Income Verification form is NOT NEEDED for Renewal.
4. Submit the completed EdChoice Renewal form AND Proof of Residency to Mr. Crawford, the SMS Scholarship Coordinator, via email ([email protected]) or in person to the school office.
